Nalgonda's Response to Theme- DAPCUs and F-ICTCs

Facility integrated counselling and testing centre is a place where a person is counselled and Tested for HIV, on his/her own free will or as advised by a medical provider. 

The main Objectives of F ICTC includes;

Early detection of HIV, Provision of basic information on modes of transmission and prevention of HIV/AIDS and Link people with other care and treatment services.

Each primary health facility should have one ICTC for all groups of people. The FICTC located in facilities that serve specific categories of people such as pregnant women, Accordingly the FICTC located in the grass route level primary health centres and private maternity hospitals where the majority of clients who access counselling and testing services of pregnant women. The FICTC consisting staff nurse, Lab technician and pharmacist and they performing test for diagnose of HIV and referring the clients to nearest ICTC for confirmatory test and the need for providing prophylaxis to prevent the transmission of HIV from infected pregnant women to their infants.  

The role of DAPCUs in establishing F-ICTCs;

Identify the existing PHCs in the district, enquire about the staffing position like Doctors/Pharmacists/Staff nurses/ANM & lab technician. Ensure the lab-facilities and ANC flow of the facility also ensure the distance from the nearest ICTC
Then the DAPCU is communicated DM&HO regarding establishment of new FICTCs and as per the directions of PD, APSACS the official G.O copy with all FICTC related infrastructure developed with the support of concern facility medical officer.
Capacity building to the F-ICTC staff (Training on maintaining the registers, records and submission of monthly reports etc.)

During capacity building the existing staffs like ANM, Staff Nurse and the Lab-technician were involved and provided training on FICTC functionaries services by DAPCU. Also gave training in the District level for the above staffs on CMIS. According the supply chain management system, DAPCU provided the registers, all types of reporting formats and testing kits also. Finally the DAPCU is co-ordinating with NRHM regarding supply of other equipment for laboratory.

Strengthening of linkages between F-ICTC and ICTCs

DAPCU is strengthening the co-ordination by timely supplying the required logistics and deputing the nearest ICTC Counsellor and LT twice in a month to visit the FICTCs for proper support and guidance. Standalone ICTCs are playing an active role in strengthening of FICTCs. The concerns ICTC Counsellors are participated in cluster level meetings and coordinating with Senior Public Health Officer (SPHO) and concerned FI- ICTC medical officer for strengthening the FICTCs. Also coordinate with RNTCP staff for cross referral.

Monitoring and evaluation of F-ICTCs

The DAPCU team, Addl. DM&HO, DPM, DIS and the nearest ICTC counsellor are regularly visiting the F-ICTCs and monitoring the centre performance and if any gap is identified in the centre, the concern standalone counsellor or concern cluster senior public health officer (SPHO) is immediately suggested by DAPCU to take actions for minimize the gap and also provide proper support and guidance to the F-ICTC. 
Future Plans

1.Planning to conduct training to all FICTCs staff nurses on SIMU package
2.Send the details to APSACS on remaining 8 new FICTCs out of 72 for universal centre official code
3.Planning to conduct meeting with all SPHOs to review the performance of each FICTC and orient on new PPTCT drug regimen program in the district.

DAPCU Nalgonda Response to DCC for HIV-TB

The overall objective of the DCC District Coordination Committee for TB/HIV is to ensure the implementation of TB/HIV collaboration activities in the District and review the performance of the same.

In this context Nalgonda DAPCU has been conducted the DCC meeting on 26th of October 2012 under the chairmanship of District Collector. During this meeting all the line department staff of DM&HO, DTCO, Addl. DM&HO, DCHS, DPM NRHM, ART SMO, and President DLN invited for sharing their views and sort out the complications came while working.

The main agenda of the HIV/TB DCC meeting is

Ø  Performance review ICTC/DMC level
Ø  Line List preparation
Ø  ICTC to RNTCP referrals review Vice-versa
Ø  Discussion on HIV/TB co infected cases pre ART registration
Ø  Discussion on coordination between ICTC to RNTCP
Ø  Sharing of experiences

TB is the most common opportunistic infection in people living with HIV. As HIV breaks down the immune system, HIV- infected people are at greatly increased risk of TB. TB in turn accelerates the progression of HIV to AIDS and shortens the survival of patients with HIV infection. Thus, TB and HIV are closely interlinked

In order to provide HIV testing facilities for individuals diagnosed with TB as well as to understand the TB status of HIV positive individuals it is important promote coordination and information sharing between DMCs and ICTCs. The in referrals from RNTCP to ICTC over the past

It was observed that, there is a gap between cross referral of RNTCP to ICTC vice versa. Hence we noticed these many issues in the DCC meeting, and the DAPCU is planning to take up special drive on HIV/TB intensified case finding in the coming months.

Having a holistic campaign by DAPCU and DTO to address the issue of low cross referrals between the HIV and RNTCP facilities by increasing the outreach combined with the simple messages with the support of the line departments

Conducted training to DMC LTs on HIV testing preferably SD screening Test. During this 28 DMC LTs have been trained on whole blood testing & counseling.
DAPCU has taken the line-list of LFU/MIS patients from ART and segregate it geographically to each NGO. The review of this activity conducted in each month in ART/CCC coordination meeting

DAPCU raised a concern about the entry in HIV-TB ART TB treatment should enter correctly on white card.  After testing of TB patient, the HIV status should include in the TB register.

In Intensified TB care finding every facility should develop good rapport with each OPD. Maximum referrals from OPD (Acute respiratory tract infection) Cough with expectation referred for TB Screening.

The District Collector instructed to DM&HO, to address the issues related to HIV/TB co-infectious cases by visiting the field and interact with the patients. According to this, instructed to DTCO and DAPCU to plan it as early as possible. 


Discussed about the X-Ray machine, it is not functioning properly in DTCO Office, in this context the District Collector suggested to DCHS to pursue the issue and submit a report regarding this. Regarding X-Ray machine the DTCO told that, it is an old model machine and not showing the proper result, hence the cases are referring to the district hospital located in the same premises because there is an advanced technology X-Ray machine for accurate result

WORLD AIDS DAY-2012, NALGONDA

          The DAPCU, Nalgonda has successfully celebrated the WORLD AIDS DAY 2012 celebrations at Nalgonda as this year theme is "Zero new Infections" "Zero AIDS related Deaths" and "Zero stigma and discrimination”. To reach this theme we organized a Rally with the Flag off by District Collector and participated, NCC Commanding Officer 31 AP Battallion, DM&HO, DCHS, RMO, Superintendent of DHQ, Nalgonda, DTCO, IMA president, IRCS president, Lions club chairmen, TI NGOs/CBOs and +ve Net works etc.
          The World AIDS Day was observed on 1st December 2012 at Nalgonda by DAPCU team. The event started with a rally gathering of at around 1000 people at the District Head quarters Hospital and culminated at Nagarjuna Govt. Degree College.


   The District Collector & District Magistrate Sri N.Mukteswara Rao IAS flag off the rally at District Head quarters Hospital by 9.30 AM. Before inaugurate the rally they both had interaction with the Print & Electronic Media and delivered their message to the public through the media on the objective and importance of World AIDS Day. The Dist.Collector also involved in the rally and encouraged to all the participants with HIV/AIDS related Slogans.
          The rally has lead by the DM&HO, ADM&HO (A&L), DTCO, IRCS president, DPM and IMA President. During the rally DAPCU staff, NSS, NCC, Bharath Scout candidates, Nursing students, Govt. Boys Junior College students, NGOs, CBOs & TINGOs, District Positive Network, SHGs, Media persons and the General public were participated.
          The rally comprising around 1000 participants, having pla-cards in their hands and with display of banners preceded to Bus Stand, Clock Tower Center and Police pared ground and end up at Nagarjuna Govt. Degree College. Throughout the rally the police department has given full support to go smoothly without creating any traffic problems. 
          In addition, many other rallies are also separately conducted in the town and merged into the main rally. In Nalgonda IRCS Voluntary Organization conducted a rally with school children and it was inaugurated by President IRCS and they went up to Nagarjuna Govt. Degree College for participating in public meeting
          The pamphlets, Hand outs and booklets are distributed in the rallies and these Rallies are created a tremendous awareness in the society and the print & electronic media covered these rallies.
          After Completion of the WAD Rally, all the participants are reached to the public meeting venue at Nagarjuna Govt. Degree College where the World AIDS Day program was organized. In the beginning of the program the Cultural activities are conducted by Govt. Boys Junior college staff & students. Then the program presided by Dr.P. Amose DM&HO. The Chief Guest of this program was Mr. Swamy Subedar, Commanding Office, NCC, Nalgonda.
          The DPM has given the introduction notes about the World AIDS Day and its objectives & importance then after he explained about the various HIV/AIDS services that are rendering in the district and its progress. All the dignitaries on the Dias delivered their speeches on WAD Vs HIV/AIDS related issues in various angles. The DM&HO delivered his valuable speech and said HIV/AIDS is not only a health issues but also a development issue and there is a need to reduce the stigma & discrimination in the society towards the HIV positives. Finally, the awards are given to the various HIV/AIDS service providers in the district by the dignitaries on the Dias, based on their performances.  Before   concluding the meeting DPM has get hold of pledge from all the participants to reach the theme objective in the future and the end of the program vote of thanks said by DAPCU, Nalgonda.
         On behalf of WAD, on 2nd December a 7 km run organized by KIMS (Kamineni Institute of Medical Sciences) with the support of DAPCU by  involving 300 medical, dental and nursing students of Kameneni Institute of Medical Sciences, Narketpally, Nalgonda Dist from District Hospital to RHTC (CCC) Cherlapally, by involving honorable district collector and government officials. The run inaugurated by Sri. N.Mukteshwara Rao IAS District Collector, Nalgonda and it was started at District Hospital Nalgonda and concluded at Community Care Center (CCC), Cherlapally. During the run the participants will disseminate the information on HIV with slogans, placards, pamphlets and banners.
Also organized a voluntary blood donation camp by DHQ, Blood Bank at Neelagiri Degree College, Ramagiri, Nalgonda, collected 34 units of blood and all the students were supported with IEC materials on HIV.

Nalgonda , Andhra Pradesh Response- National Voluntary Blood Donors Day

Observed National Voluntary Blood Donors Day 2012 at DM&HO Office, Nalgonda on 1st October 2012 as the theme of this year is ‘SOMEWHERE SOMEONE NEEDS YOU. YOUR BLOOD DONATION IS PRECIOUS’. We emphasized on creating awareness
  • on voluntary blood donation
  • and strengthening the partnership with local bodies to appreciate with Best Donor awards.
Before this event also the DAPCU, Nalgonda has done more publicity to promote the blood donors.
Blood donation is an essential component of health care; it contributes to saving many of lives each year in both routine and emergency situations, in medical and surgical interventions and improves the quality life of patients in acute and chronic conditions.
The program was started with the ribbon cutting by Dr. Suresh Kumar, DCHS for the seminar cum blood donation camp hall. Then organized a seminar on this occasion and the respected dignitaries like, DM & HO, Medical superintendent, IMA President, RMP Association state president, Lions Club Regional President and Addl.DM & HO (A&L) have been spoken about the importance of the occasion.

We recognised a few people on this occasion 
Before completion of the seminar,  10 members were selected for best motivators, 5 regular blood donors and provided awards with appreciation certificates. Best Motivators and Donors were those who motivate the donors and donate blood since last 3 to 4 years. After award distribution Dr.Suresh Kumar DCHS, Nalgonda has started the Blood donation camp. During this at around 40 units of blood have collected and planned to organize more camps within the month. The entire program was coverage by local city cable network and press media partners. In this occasion involved local youth associations, TI NGOs and Link Workers, etc.

Other Blood Donation related activities
Apart from as part of the Rakthabandhu campaign during the event of Police Commemoration Day the DAPCU Nalgonda has conducted a voluntary blood donation campaign (21.10.2012) at District Police Pared Ground, Nalgonda. Before starting the campaign the respected Superintendent of Police, Dr. Naveeen Gulati, IPS has given opening speech on importance of the day and appreciated the team of police constables for willing to come forward to donate the blood, then the camp has inaugurated by respected SP with ribbon cutting of the campaign.
           
In this program CH. Vijaya Rao DSP, Mr. Srinivasa Rao Traffic CI, and 5 members of Sub Inspectors and 100 members of police constables participated actively and donated 70 units of blood. Organized sensitization program on blood safety and HIV prevention with the above mentioned participants and all the participants were supported with IEC materials.
