Visit of US Ambassador, Visakhapatnam District, AP

US Ambassador visit to DAPCU, Visakhapatnam, AP

              District AIDS Prevention and Control Units are started in March 2009 and received many trainings to perform their duties in HIV Prevention activities. CDC and Share India supporting the DAPCUs in trainings. In his visit to Visakhapatnam U.S. Ambassodor want to see the performance of DAPCUs. 
              To support this visit Dr. J.C. Reddy, JD,BSD, APSACS, Dr. Prabhakar, SPO, Share India, Sri Ramesh Babu, CDC visited the DAPCU ,Visakhapatnam.28-1-14:

              King George Hospital (shortly KGH) is a Government General Hospital located in Visakhapatnam, Andhra Pradesh, India. The hospital served the needs of north coastal Andhra Pradesh and adjacent Orissa for more than 150 years.It was started as civil dispensary in 1845 and upgraded into a 30 bedded hospital in 1857. In 1923 started as King George Hospital and expanded in 27 acres and affiliated to Andhra Medical College. 250 multi speciality blocks with 1023 beds serving the poor patients from neighboring districts as well as states.

        Mr.Michel Mullins interacted with Counsellors and LT, Dr.J.C.Reddy explained about the ICTC services. Patient flow,referral services and types of counseling explained by the staff. Team visited the ICTC and ART labs. Medical officer explained about the functions and daily work load etc.
               After that a small meeting was organized in the seminar hall. American Consul General and his team interacted with DAPCU,ICTC and hospital authorities.  Medical Officer from  ICTC and SRL  Dr.Ramana and Dr.Kalyani, presented their center performances.

.Explained the prevalence ,new PPTCT programe &results,DAPCU roles and responsibilities,major thrust areas and challenges.

           Sri Michel Mullins asked about the steps  taken to target the 15-35 age group. DPM explained about the mainstreaming activities like AEP,RRE and Police sensitization programmes and the results. The team has visited the photo exhibition and Medical officers explained about the activities.
ART Center, KGH:

ART Center was established in January 2005. At present (as on 31, December 2013) there are 14,654 People Living with HIV/AIDS (PLHIV) got registered for Pre-ART at this center. Of these, 5058 PLHIV are alive and on ART taking medicines. This Centerprovides ART services of 1st line, 2nd line and also has pediatric ART services provided to CLHIV.


The team has visited the ART center. Staff nurse explained about  the  Pre ART registration. Dr.Prathap Reddy,SMO,ART center explained the services available in the ART center. 

 Later he interacted with the ART patient . Her husband is HIV negative and she delivered the child . Her child is recently tested negative for HIV at hthe age of 18 months. She is following the ART and satisfied with the counseling at ART center. 

           Mr.Michel Mullins and his team satisfied with the services at ICTC and ART centers. He appreciated the APSACS,CDC and DAPCU team for their efforts in HIV /AIDS activities. DPMs from Vizianagaram and Srikakulam also participated in the visit. 

          On behalf of DAPCU Dr.Madhusudana Rao,Superintendent,KGH has presentated a momento to Mr.Michel Mullins for his visit to Visakhapatnam to observe the HIV/AIDS prevention activities.

The main advantages of DAPCC meetings                                                                                                           

DAPCC Meetinf
  • This creates a single  platform  for discussion of HIV related issues with other stakeholder department 
  • It provides strengthening of processes of  support and benefit schemes to PLHAs in other departments 
  • Awareness about the services and facilities to other stakeholders helps in coordinating the programmes conducted by DAPCU.
  • Certain   administrative issues at some institutions which takes longer time to get solved can be speeded up with the directions of DC or CEO
DAPCC in Dakshina Kannada district is conducting regular meetings under the chairmanship of DC and ZP CEO. Many issues solved under DAPCC like
CEO Distributing Loans
  • Social benefit schemes for PLHAs and FSWs was processed and 6 PLHAs received self employment loans from Backward and Minority Corporation.
  • Housing schemes applications from some PLHAs received   and is under process by taluk       panchayath officials.
  • There was an issue of doctors not following NACO   guidelines of breast feeding by positive mothers. This issue was brought to the notice of DC at DAPCC and DC instructed the Administrative Officer to resolve this concern. A  meeting   was called and for sensitization for all the doctors and to discuss the issue. 
  • Labour department in coordination with DAPCU organized HIV/AIDS awareness camps for workers at some industries in the year 2010-11                                                                                                                            
  • Legal department   along with lawyers bar association organized a sensitization programme for lawyers in coordination with DAPCU and one positive person nominated as the honorary   member of the association to solve any legal issues of PLHAs

Advantages of DAPCC Meeting
  • DAPCC gives opportunities for better co-ordination with various departments. After DAPCC meetings the process of social benefits schemes has become easier to certain extent.
  • Through  DAPCC meetings, the DLN and PLHIV have been motivated  to apply for schemes (taluka wise)  &  follow up  with  concerned  officers to  sort out  the problems in sanctioning  the social  benefits schemes.
  • In DAPCC meeting a emphasis  was given  over the follow up of the PLHIV  who are on ART.
  • Other Departments agreed  to help in detection of HIV  from various sectors like Labour organisations, Migrants etc.

 Achievements
  • 100% coverage of ANC for  HIV testing become much  easier  [Target - 4776, Achievement- 5457 (114%)]
  • HIV Testing in General clients has been increased - [Target -5844, Achievement -8145 (139%)]

Peren DAPCU's Response to May 2012 Theme - DAPCC Achievements

Benefits of DAPCC meeting
  1. DAPCC meetings give platform to DAPCU and the DLNs to sensitize HODs of various departments about HIV/AIDS issues and scenario in the district.
  2. Face-to- face sittings with other departments and stakeholders in turn helped DAPCU to voice its concerns and urge them to act. Some departments as well as organizations have started to include HIV/AIDS programs in their own departments with the help of DAPCU.
  3. DAPCC meetings have helped DAPCU and the DLNs in familiarizing and building rapport with key persons in other departments/ organizations.
  4. Other departments and organizations have assured in helping to reduce the epidemic and helping the PLHIV in their own capacities.

Some Achievements supported by DAPCC
DAPCU’s initiation to converge with stakeholders and other departments has been fruitful. Slowly, departments and even organizations like the church and students/village youth organizations have started to approach DAPCU to be resource persons or to conduct programs for them. A few examples

  1. Sensitization program with police force and their wives at New Reserve, Peren: A sensitization program with police personnels and their wives at New Reserve, Peren was conducted on 6th March 2012. The program initiated by DAPCU together with the Superintendent of Police, Peren. The program was attended by the SP, Addl. SP, other police officers along with the police staffs posted at New Reserve, Peren. Altogether, there were 82 participants who attended the program.
  2. HIV/AIDS awareness conducted at Men’s Department, Jalukie Town Baptist Church, 28th March 2012: Mr. Ngauzeulungbe M&E (Asst.) DAPCU, Peren gave a power point presentation in which he shared what HIV/AIDS is and how it is spread. He shared about the brief history of how HIV/AIDS was first detected.  He also shared about the various route of HIV/AIDS transmission, and highlighted about how Blood Transfusion, sharing of Needle & Syringes and Mother to Child can cause HIV/AIDS. He shared on Stigma & Discrimination, how it is one of the main reasons for the spread of HIV and AIDS and how we need to care for the positive people and help them live a normal and responsible life. He urged the participants to educate their near and dear ones about HIV/AIDS and also help to eradicate stigma and discrimination in the society.
  3. FGD conducted by NRHM (Convergence activities with NRHM): Four FGDs were conducted at Peren Town by NRHM from 22nd- 23rd March 2012. M&E (asst) of DAPCU peren was the resource person. The FGD was focused on STI/RTI and the participants were mainly women folk comprising of lactating mother’s, pregnant women, newly married and stakeholders. The M&E(asst)not only stressed on STI/RTI, pre-natal care, child care but also talked on the basics HIV/AIDS and stigma and discrimination. The participants were responsive and many queries were clarified.

  2. HIV Awareness and Blood testing at Sub-jail, 24th January 2012
  3. Sensitization Program conducted at Tening Block with Legislatures Forum on HIV/AIDS (LFA) on 12th May 2012

Bellary DAPCU's Response to May 2012 Theme - DAPCC Achievements

In Bellary District, District AIDS Prevention & Control Committee has been formed on 02.06.02011 with the Chairmanship of Deputy Commissioner. In the committee we have discussed about the incidence, prevalence, services and social entitlement programme (for PLHIV’s eg: Housing scheme, Anthodaya Anna Yojana scheme, vishesha palana yojana-OVC, Bhagyalaxmi yojana, Loan schemes etc).

Chairman (Deputy Commissioner) / Co-chairman (Chief Executive Officer, Zilla Panchayath) suggested to build awareness in the uncovered area’s or people living in such areas are not able to come forward to seek ICTC services eg: Central jail, factories, industries etc. and also instructed the other line departments to co-operate in bringing down the fresh infections and simultaneously provide social entitlements for the needy PLHIV’s as per the government guidelines.

As per the committee suggestions we have implemented the awareness / testing programme in uncovered areas. First we have planned to organize the same in Central jail with the help of Jail superintendent Mr P V Anand Reddy and Assistant superintendent Dr Ranganath. After their permission 2 programmes were organized in the central jail on Sundays with the help of ICTC team for prisoners. (programme include documentary film on Stigma / discrimination and on OVC, awareness on HIV through PPP, then followed by counseling and testing including Blood grouping). During this month (June 2012) we are planning to conduct the same kind of programme in Women Rests Home attached to Women & Child Welfare Department (i.e on 13.06.2012) and garment industry (i.e on 16.06.2012) and in future months we are going to plan to conduct the same Rice mill workers, Local Urban body employers-workers (corporation/city municipality etc).

Finally with the formation of committees (DAPCC, Task Force Blood Safety, HIV-TB Co-ordination committee) helped us in implementing the programmes in smooth & broader way. And the results our evident in our performance.

In the DAPCC meeting Deputy Commissioner instructed to all the taluk health officers to conduct VBD camps in various college / institutions / NGO’s / industries . As per direction by DC from DAPCU office prepared the 3 months micro plan for VBD camps, as per dates PHC Medical officers conduct the VBD camps. Now monthly VBD camps micro plan giving to all the PHC/GH from DAPCU office. Compared to past 4 years VBD camps & Blood Units increases in Bellary Dist. 


After the committee formation the  Deputy Commissioner discussed reducing  positivity  in Bellary and instructed  all the Taluk Health Offcers to achieve the target and give the HIV/AIDS awareness programmes to all institutions / organizations. 4 years HIV testing and positive details attached in the slide.

Araria DAPCU's Response to May 2012 Theme - DAPCC Achievements

DAPCC Advantages
  • DAPCC formation is helpful for strengthening to the DAPCU &helpful to convergence with NRHM for awareness and demand generation at facilities is concern.
  • DAPCC help to provide a platform to integration with government department and other line department for mainstreaming of HIV/AIDS is every one’s accountability.
  • DAPCC provides to supporting for integration of PLHIVs with government schemes, regarding care and support to PLHIVs.

Achievement
Through the DAPCC we identified 4 additional schemes which include MNREGA, and widow pensions schemes. The concerned officials were sensitized through the DAPCC.


Champai, DAPCU's Response to May 2012 Theme - DAPCC Achievements

While preparing for the DAPCC meeting dated 22nd July 2011, DAPCU team believed that it is a great platform to inform, enquire and request each concerned Departmental head/officers from the DAPCC members about schemes or benefits which can be made available for our PLHIV from the District. And for this, the DAPCU team looked into various schemes under each department and, these were presented as PowerPoint presentation on the DAPCC meeting, and discussion after each departments were held on making these available for PLHIV. The DAPCU was also able to work with various other populations with the help of the DAPCC. A few examples 


CAMPAIGN AMONG DISTRICT JAIL INMATES:
Meeting with the Superintendent, District Jail on the 28th July for Jail Visits
DAPCU visited the District Jail twice, first with Mobile ICTC, the later with ART Centre
  • 18th August: Basic knowledge on HIV/ AIDS was delivered to about 100 inmates. Followed by HIV testing. 27 inmates were given one on one counseling and tested. 2 tested positive for HIV.
  • 15thSept: Care Support and Treatment for PLHIV particularly on ART, CD4 counting and Criterion for ART was presented. After which CD4 counting was done for 3 inmates. One old and 2 new case that were detected on the last visit and Pre ART done on the spot.
 AEP School Programme

  • DEO, Champhai district issued a circular to all schools in the district for utilizing the first Monday of every month for AEP, which includes topics on HIV/AIDS, Drug abuse and Sex education.
  • DAPCU prepared uniform PowerPoint presentation on ‘Basic knowledge on HIV/AIDS’, which were distributed to the 19 prioritized schools.
  • On following up on the progress on AEP, in most of the schools, 36 (Thirty six) Nodal teacher were identified and trained. And except for few schools, this is an ongoing process and 36 (Thirty Six) schools report they have it regularly and successfully. 
SCHEMES
Civil Supplies

AAY: Meeting with DCSO was held  4th August .A list of PLHIV who are willing to access AAY card were given. And 19 PLHIV were entered. 21 PLHIV enjoying AAY card in the District at present

Social Welfare Department
  • Nutritional supplements to PLHIV DIC and ART Centre: – Malwin, Cereals, Soya, Calcium Tablets, Biscuits etc are frequently supplied to our centres from CDPO, Champhai.
  • Lists of PLHIV were collected and application made for Indira Gandhi National Widow Pension Scheme (3 PLHIV) and National Family Benefit Scheme (8)

HIV/ AIDS messages in Conferences and Churches: A circular was issued to all departments by the Deputy Commissioner, Champhai, to include a short session on HIV/AIDS awareness in every Conference and meetings within the district. Circular on HIV /AIDS information’s and messages issued to all the Churches in the District which were read out during services.

What is DAPCC?
DAPCC (District AIDS Prevention & Control Committee) is a district level association of district level officials of line departments to observe and co-ordinate the HIV/AIDS activity in the district and ensure for smooth implementation of different activities through District AIDS Prevention and Control Unit (DAPCU).

  2. ADMO (PH) or DACO-Member Secretary, District Programme Manager, DAPCU- Member

  • Raising the various issues concerning to HIV/AIDS in the district and action taken for such issues by interaction and coordinated effort from all line departments and the members of the DAPCC

Achievements through DAPCC

  1. 100% ANC screening for HIV/AIDS at the health institutions: As per decision taken in the DAPCC meeting, an order has been issued from CDMO/ District Collector for 100% screening and it became mandatory during ANC checkup.
  2. Provision of free investigation and blood transfusion to the PLHIV- As per this the cost towards treatment and blood transfusion to the PLHIVs are made free or without any user charge.
  3. Provision of additional remuneration to ASHA for institutional delivery of HIV +ve pregnant mother by accompanying her: It has been decided and corpus fund supplied fund to DAPCU for providing additional remuneration i.e. Rs.1000/- to ASHA for accompanying a +ve ANC cases to the heath institution for delivery. 
  4. Inclusion of HIV/AIDS components in NRHM as well as other line departments: The IEC leaflets has been prepared and distributed at the block level during the programme conducted by NRHM through PHC/ CHC. At the district level DAPCU also involved in various activity or programme conducted by NRHM and disseminated information on prevention and control of HIV/AIDS.
  5. Conduction of IEC activity by  NGOs at the Borigumma and Kotpad by keeping in view of vulnerability on HIV/AIDS as well as STI.
  6. Provision of TA for PLHIVs coming to ART centre and the expenditure was met from Corpus Fund of Red Cross Society of Koraput district

Bangalore Rural DAPCU's Response to May 2012 Theme - DAPCC Achievements

We are conducting DAPCC Meeting monthly along with the District Health Mission Meeting in Bangalore Rural District. All the programme Officers (RCHO, DTO, DPMO, DMO, DLO, DNO) are attending the meeting, even though they are not members of DAPCC. NGO representatives and all the line departments are called for the meeting. It is going to be useful to take decision on any issue at District Level.

Advantages and Achievements
  • Coordination with other line departments and provides solution to each problem
  • DAPCU facilitates mainstreaming of HIV/AIDS program through DAPCC meeting.
  • The District Administration/DC taking appropriate action towards HIV prevention & support to the programme
  • Voluntary Blood Donation Camps conducting, in collaboration with other departments.
  • Around 80 OVC children list submitted to Women and Child Development Dept. for school scholarship and other benefit.
  • 100% achievement of ANC positives to reach  Pre-ART Reg. CD4 cont& followed-up.
  • Pre-ART Reg. & CD4  of general clients who were diagnosed positive was improved from 45% to 80%

Bhiwani DAPCU's Response to May 2012 Theme - DAPCC Achievements

DAPCU Bhiwani started its DAPCC on 18-4-2012 under the chairmanship of then Deputy Commissioner Sh. Nitin Yadav and Member Secretary Dr. Manish Kumar. DDPO, CDPO, District Labor officer, District Mining Officer are the Nodal Officers of DAPCC from other departments. The constitution of DAPCC proved very helpful in working of DAPCU. 

BPL Ration Cards
The DDPO and CDPO and District Welfare Officer were contacted for information on welfare schemes of their departments. Following which the DAPCU made a list of PLHA who have not benefited by these schemes
  • 138 PLHA are found needy for BPL card out of 169
  • 31 PLHA are those who already have BPL cards or are not interested in the scheme
  • Demands were taken from these PLHAs and DAPCU started a PUC to make available BPL (Below Poverty Line) Ration Cards. 
IEC through Municipal Corporation
  • There was a grant at Municipal Corporation, Bhiwani for some publicity and display of Govt. welfare schemes  on the ring road side of city. 
  • DAPCU contacted to the concerned persons and was able to convince them to display the HIV awareness boards. Two boards were displayed on the frame of Municipality Corporation. This was all done at no cost to the programme.

Issues/problems of PLHA
Issue/problem of PLHA will be solved in the coordination meeting of ART, CCC and DIC
·   SMO ART, Centre is conducting coordination meeting among ART, CCC and DIC for solving of Issue/problem of PLHA every last month.
·   348 nos of on ART clients have been given travelling cost of ART centre @ 200/- per visit.
Reach more of PLHA under Mo Kudia Scheme.
DPM-DAPCU will consult with DRDA section regarding this matter.
·   49 nos. of PLHAs have applied to PD DRDA to include them in Mo Kudia Yojana.
IEC Hoardings in all Industries and Corporate Houses.
Letter will be issued to all industries regarding installation.

·   Hoarding installed in two industries IMAMI and Birla Tyre.
HIV/AIDS Prevention, care & support activities should be included in Village Health Action Plan.

·  Coordination has been developed with CDPOs, BDOs & Other Govt.  Officers regarding on HIV/AIDS issue.
HIV-TB Coordination Meeting:
District TB Officer reviewed the entire HIV/TB collaborative activities.
After through detail discussion, it was observed that the cross referral is 80 to 90 %, where both DMC and ICTC is available. But the referral is Zero where ICTC is not available.
Letter will be issued to all MO I/cs for 100 % referral of TB cases to ICTCs.

·  TB cases of DMC Hatigarh (where there is no ICTC) are undergoing HIV testing at ICTC Jaleswar.

Balangir DAPCU's Response to May 2012 Theme - DAPCC Achievements

Advantages of having DAPCC in the district
DPM appraising the Collector and other members of DAPCC
  • Communication of all HIV related programmes to all departments  in one common platform.
  • Presentation on severity of HIV/AIDS situation in the district to the apex body of the district for early and instant action.
  • Best platform for convergence activity with NRHM and other line departments.
  • Creates a good environment for mainstreaming.
  • New plans and proposals are taken into immediate decision as the District Collector is the Chairman and supreme authority of the committee and presides over the meeting.
  • Reduces time consumption for resolution of critical decision for programmatic development
Major achievements through DAPCC meeting.
  • PLHIVs are getting their traveling cost (to & fro) from their block head quarter to ARTC. In addition to this Rs. 40/- additional amount is given. This cost is borne by Red Cross fund. In turn this will reduce the LFU / MIS cases and increase re-enter of clients to ARTC.
  • As the Lab. Tech. posts are lying vacant in 11 stand alone ICTCs, HIV test in those ICTCs is done by by deputing OSACS Lab. Tech. and by Govt. LTs trained for HIV testing . In this process during March 2012 we have made  13 ICTCs functional with only 5 OSACS LTs. The TA/DA of concerned deputed LTs, is being paid from the local (where the LT is deputed to conduct test) RKS fund as per state Govt. norms. It has been also decided to provide Rs.1000/- as incentive where HIV test trained existing Govt. LTs are available to make the  ICTC functional.
  • Free medical tests for PLHIVs at DHH and SDH pathology.
Mass awareness campaign on the eve of Balangir Lok Utsav

  • Emergency financial help for better treatment is being given to PLHIVs through district Red Cross and they are also being considered for Odisha treatment fund.
  • Through DAPCC, it is becoming easy to coordinate and plan successful VBD camps at different levels in different time involving all related stakeholders. On special occasions like National Youth Day, Red Cross Day and Anti Terrorism Day etc. we are planning mega voluntary blood donation camps with IPC sessions.
  • Expenditure for critical HIV patient’s referral to different Medical Colleges of the state for availing higher medical service, is being met from RKS fund.
  • Tracking of Out Migrants for destination oriented intervention for their betterment and provision of IEC and CST is being done through Gaon Kalyan Samitee (GKS).
VBD during Anti Terrorism Day
  • Free Mosquito Nets (Long Lasting Insecticidal Nets) to all PLHIV who undergo Pre -ART registration.
  • Double nutritional food to all 19 CLHIVs through ICDS.
  • Financial help through INP+, HIVOS & Concern Worldwide in shape of loan (interest free Rs. 10,000/-) to 42 PLHIVs.
  • Various social benefit schemes are being disbursed to PLHIVs with an easy established process under the direct supervision of DAPCC Chairman.
  • Ensured legal assistance in case of social rejection, property/ responsibility denied, as well as stigma discrimination as and when reported.
  • Ensured several mass awareness campaigns with sharing the platform of district administration along with other line departments and socio cultural activities organized by public private partnership / involvement in most of the district sub-district level events.
